Perfect symmetry is a graphic-design concept, not a rule of human anatomy. One eye opens a little wider. One foot can be larger. One testicle commonly hangs lower or feels slightly different from the other.

What you are actually feeling

The scrotum contains more than two smooth balls. Each testicle has an epididymis, a soft coiled structure that sits along the back and upper area and stores and carries sperm. There are cords, vessels, skin, and tissue. First-time self-checkers sometimes discover the epididymis and assume they found a tumor.

One side may hang lower, one testicle may be a little larger, and the scrotal skin may tighten or loosen with temperature, stress, and movement. That shifting is normal.

The emergency is a sudden change plus pain

Testicular torsion happens when the spermatic cord twists and reduces blood flow. It is most common in adolescents and young adults, but it can occur at other ages. The classic warning is sudden, severe pain on one side. Swelling, nausea, vomiting, abdominal pain, or a testicle sitting unusually high can occur too.

Do not wait for school to end, sleep on it, or hide it from embarrassment. Sudden severe testicle pain needs emergency assessment now. The chance of saving the testicle is better when treatment happens quickly.

Pain can briefly improve if the testicle untwists by itself. That still needs urgent medical attention because it can twist again. Do not attempt internet maneuvers or ask a friend to “fix” it.

What about a painless lump?

A painless lump is not automatically cancer. Cysts, enlarged veins, fluid, inflammation, and other benign conditions are common. But a new hard lump inside a testicle, persistent enlargement, heaviness, or a lasting change should be checked promptly.

The point of knowing your normal is not to run a daily tumor search. It is to notice a real change and then hand the diagnosis to someone trained to make it.

A calm monthly familiarity check

  1. Check after a warm shower when the scrotal skin is relaxed.
  2. Support one testicle and gently roll it between fingers and thumb.
  3. Notice the smooth firm testicle and the softer epididymis behind it.
  4. Repeat on the other side and note what is normal for you.
  5. Stop after a couple of minutes. Repeated checking can create swelling, tenderness, and anxiety.

Things that deserve an appointment

  • A new hard lump or firm area.
  • Persistent swelling, enlargement, heaviness, or aching.
  • A change in shape or feel that lasts.
  • Discharge, burning urination, fever, or sexual-exposure concerns.
  • A history of an undescended testicle and a new concern.

Why the height changes during the day

Muscles in and around the scrotum move the testicles closer to or farther from the body. Cold, fear, exercise, sexual arousal, and touch can temporarily pull them upward. Warmth usually lets the scrotum relax. That movement can make yesterday's careful comparison look different today.

A soft “bag of worms” feeling above a testicle can be a varicocele, which is an enlargement of veins and is more common on the left. A smooth fluid-filled swelling can have other causes. These are often not emergencies, but a clinician should identify a new swelling rather than you naming it from a diagram.

After a hit or sports injury

Minor discomfort may settle, but severe pain, major swelling, bruising, nausea, an abnormal position, or pain that does not improve needs urgent assessment. Wear appropriate athletic protection for sports where direct impact is possible. Do not assume every painful event is “just getting hit in the balls.”
